Athletics Department

The Central Regional Athletics programs play an important part in the life of our students. Lessons in sportsmanship, teamwork, competition and how to win and lose gracefully are integral parts of the district's athletic program philosophy. Athletic participation is also essential to helping students develop a healthy sense of self–esteem as well as a healthy body.

Athletic participation is also essential to helping students develop a healthy sense of self–esteem as well as a healthy body.

Required Forms

If your student plays sports or is interested in playing sports, you must complete the forms below before participating.

Physical examination packets must be turned in by the following dates:

  • Fall Sports: August 10th

  • Winter Sports: October 15th

  • Spring Sports: February 15th

Family ID Program

In addition to the required forms above, you must register your student through the FamilyID program.

FamilyID is a secure registration platform that provides you with an easy, user-friendly way to register for our sports  programs, and helps us to be more administratively efficient and environmentally responsible. When you register through FamilyID, the system keeps track of your information in your FamilyID profile. You enter your information only once for each family member for multiple uses and multiple programs.

Additional Medical Forms

If your student has an existing medical issue requiring medical intervention, medication to be administered during school hours, or asthma/allergies, you will find the required medical forms here.

Out of Gym Policy

Physical Education is just as important as every other class  your student engages in. If your student needs to be out of gym, you must provide a doctor's note to keep the student out of gym. It must be updated by the doctor every 30 days. Also, if the student plays sports, they will not be able to return to their sport unless the doctor's note states "can return to gym and sports".
